Position Summary

Power Health Colorado is seeking an experienced, highly organized, bilingual (Spanish/English) Care Coordinator to serve as the primary point of contact for patients throughout their treatment journey.

The Care Coordinator is responsible for managing patient referrals, coordinating care across multiple specialties, communicating with attorneys, medical providers, insurance representatives, and referral sources, while ensuring every patient receives exceptional service from referral through case completion.

This position requires someone who thrives in a fast-paced medical environment, possesses exceptional communication skills, and can independently manage a high-volume caseload while maintaining outstanding attention to detail.


Primary Responsibilities

Referral Management

  • Receive and review incoming referrals.
  • Contact referral sources immediately to acknowledge receipt.
  • Contact patients promptly to schedule appointments.
  • Introduce yourself as the patient's dedicated Care Coordinator.
  • Coordinate appointments across multiple departments.
  • Monitor referral completion.
  • Ensure same-day scheduling whenever possible.

Patient Care Coordination

  • Serve as the patient's primary point of contact.
  • Build strong relationships with patients throughout treatment.
  • Answer questions regarding appointments and treatment plans.
  • Coordinate multidisciplinary care including:
    • Medical Providers
    • Physical Therapy
    • Chiropractic
    • Massage Therapy
    • Behavioral Health
    • Pain Management
    • Imaging
    • Outside Specialists
  • Maintain ongoing communication throughout treatment.

Case Management Support

  • Monitor patient progress.
  • Perform regular case reviews.
  • Identify barriers to treatment.
  • Coordinate follow-up appointments.
  • Request and obtain medical records.
  • Track outside referrals.
  • Monitor pending diagnostics.
  • Assist providers with care coordination needs.

Documentation

  • Maintain complete and accurate documentation in the EMR.
  • Document every patient interaction.
  • Update referral notes.
  • Record attorney communications.
  • Document provider communications.
  • Maintain detailed case timelines.
  • Ensure charts remain current and organized.

Communication

Communicate professionally with:

  • Patients
  • Attorneys
  • Medical Providers
  • Referral Sources
  • Insurance Representatives
  • Employers (Workers' Compensation)
  • Internal Clinical Teams

Respond to calls, emails, and messages promptly while maintaining excellent customer service.


Case Audits

Regularly review assigned patient cases to ensure:

  • Treatment is progressing appropriately.
  • Documentation is complete.
  • Records have been received.
  • Referrals remain active.
  • Follow-up appointments are scheduled.
  • No patient falls through the cracks.

Workflow Management

  • Maintain assigned work queues.
  • Complete daily actions.
  • Resolve EMR tasks.
  • Follow up on outstanding requests.
  • Escalate barriers appropriately.
  • Meet productivity expectations.

Qualifications

Required

  • Fluent in English and Spanish (written and verbal)
  • Minimum of 2 years of Care Coordination, Case Management, Medical Assistant, Referral Coordination, or Medical Office experience
  • Experience working within an Electronic Medical Record (EMR)
  • Exceptional customer service skills
  • Strong computer proficiency
  • Excellent organizational skills
  • Ability to multitask
  • Ability to prioritize independently
  • Professional communication skills

Preferred

  • Personal Injury experience
  • Workers' Compensation experience
  • eClinicalWorks (eCW) experience
  • Multidisciplinary medical practice experience
